What is ASQ-3?

Overview

The ASQ-3® is a set of simple questionnaires about child development. The questionnaires look at how your child is doing in areas such as language, movement and social skills. The ASQ-3® can help identify your child’s strengths as well as any areas of challenge. Early detection and intervention of possible developmental delays have a positive impact on health outcomes.

ASQ-3 questionnaires

  • are available in Arabic, English, French, Spanish, and Vietnamese

  • take just 10–15 minutes for parents to complete and 2–3 minutes for professionals to score

  • capture parents’ in-depth knowledge

  • highlight a child’s strengths as well as concerns

  • teach parents about child development and their own child’s skills

  • highlight results that fall in a “monitoring zone,” to make it easier to keep track of children at risk

  • can be completed at home, in a waiting room, during a home visit, or as part of an in-person or phone interview.

Frequently Asked Questions

  1. What happens during the screening?

    • The parent or the legal guardian will complete the questionnaire, which includes 30 questions in 5 developmental areas.  You will have time to ask questions, and the results of the screening will be provided to you before you leave your appointment.

  • If areas of concern arise, the screener will either provide you with information, a referral, or set up a follow-up visit with you. This will be discussed at your appointment.

2. What are the criteria for participation?

  • Washington State residents only

  • For children 1 month to 5 1/2 years old

  • Child must be accompanied by parent or legal guardian

  • Parent must complete demographic form at start of appointment (takes 15 minutes)

3. Where can we complete ASQ?

  • Virtually, or in person, depending on the client and case manager availability.

4. How long does it take?

  • Less than half an hour. However, if you need assistance or interpretation in another language, the appointment could go over an hour.

5. How much does it cost?

  • All screenings are free. However, if you are able to make a donation to support our agency, we appreciate it.
